TERMS AND CONDITIONS
MEMBERSHIP AGREEMENT
When purchasing the Perfect 10 Spa membership,
the client acknowledges that the terms and conditions of said membership(s) is
recognized as a binding contract between the client (also known as “Member”)
Perfect 10 Spa membership rights and benefits are to commence when a Member has
physically or electronically signed the form of the terms and conditions and
has purchased a membership.
MEMBERSHIP
Perfect10 Spa reserves the rights to:
Change terms and conditions
Adopt additional rules
Adjust regulations
Alter policies
Modify procedures
Terminate Perfect 10 Spa membership or a
Member’s membership
Changes to the membership programs, by
Perfect 10 Spa , can take place at any time without prior written notice to
Member. By participating in a membership program, the Member agrees to these
terms and conditions, rules, and regulations. Members are responsible for
remaining knowledgeable about Perfect 10 Spa terms and conditions. Members agree
to pay member fee to Perfect10 Spa Memberships. Members may request to upgrade
or downgrade their membership. If a Member decides to change their membership
level, then the remaining benefits will rollover to the new campaign.
Membership at Perfect 10 Spa entitled of
benefits are depending on package member signup. member need to fully utilize
all credits point in the account and no exchange for cash, no tranfer to other
or refund.
E-Voucher Gift

E-Voucher gift will send to purchaser or recipient email as
request from purchaser.

E-Voucher gift valid for
12 months from the date of purchased.

Non-refundable or re-issued if lost / misplaced.
Membership Validity
Validity period 24 months from the date of sign-up
Treatment Selection
Member can select any treatment of choice
(Subject to change and without period notice)
Membership Limitations
Discounts on products
and tools apply to in-store purchases only. Discounts and membership are
non-transferrable and can only be used by Member. Members must provide their
name and identify themselves as a Perfect 10 Spa Member making payment. Perfect10
Spa reserves the right to request proper identification from a Member
before providing discounts or deduct credit point from member account. In order
to receive a birthday discount, Member must provide a date of birth when
signing up. Birthday discount may be redeemed 10 days before, the day of, or 10
days after Member’s birthday. Birthday voucher will expire if not used
within the allotted time-frame
The terms and conditions contained herein
shall govern the relationship between the Customer and Perfect 10 Spa,
relating to the service and products provided by the Company and all relevant
aspects that arise during the period of services provided within the outlets
and/or with contract agreement between the customer and the company.
CUSTOMER’S OBLIGATION
The Customer shall provide the Company with full cooperation and
such information concerning the Indemnity declaration form, health conditions,
allergies and agreed to accept after treatment effect which may occurred. The
customers’ health conditions and all related records and such other information
and cooperation which the Company may deem necessary and may reasonably
request, in order to enable the Company to perform the Services.
The Customer shall clarify the cause and after effect of
treatments as advice and provided by Perfect 10 Spa before
proceed with any treatment.
The Customer shall remove all items and articles of value and of
importance during the term of services and store in areas designated by the
Company to avoid any subsequent dispute arising from the loss or misplacement
of such items or articles. The Company shall not be liable for any loss or
misplacement of any such items and articles.
EXCLUSION AND LIABILITY
Please note that wellness treatment aims to
provide improvement to your current skin/health issue(s), and the outcome of
treatment is subjective. Results may vary between individuals.
All products used or to be used in the
Company’s services have no known side effects but in the unlikely event of skin
irritation due to your own allergies/sensitive skin, or appearance of redness
or bruises due to the customers’ own physical body constitutions, the Company
will not be held responsible.
DISPUTE RESOLUTION
Any dispute arising out of or in connection with
this agreement shall be first reported to Perfect 10 Spa Customer Care for
resolution through email at perfect10bodyandcare@gmail.com or contact-8666 8919
customer service during working hours 11.00am - 5pm.
The Company reserved the right to pursue against
defamation from customer in any forms which directly and/ or indirectly causes
loss to the Company’s interest and reputation.
CONTACT
Members agree to allow
Perfect 10 Spa to communicate via email, call, text, or through external
websites. Perfect 10 Spa may use these channels to communicate with Member
about account status, membership changes, altered policies, special offers,
promotions, or other matters Perfect10 Spa believes may be of interest to Member.
